Our experience tells us that when a patient feels uncomfortable with the shape of their nose their self confidence can be affected. One of the most common fears is to not be able to imagine what your nose will soon look like. For this reason it is very important to do a careful analysis and prepare an individual operative plan (no two noses are the same) to communicate to our patients what is, and what is not a possibility when carrying out a Rhinoplasty.
What is Cosmetic Rhinoplasty?
Rhinoplasty is a term which describes a series of surgical procedures with the intent to modify the shape of the nose.
Primary Rhinoplasty is carried out on noses operated on for the first time. Secundary Rhinoplasty or revision is when it is carried out on a nose that has been previously operated on.
The modern Rhinoplasty works to achieve a natural inprovement of the appearance of the nose and preserve the nasal respiration.

What does a Rhinoplasty achieve?
We can make the nose straighter, reduce a lump or reduce a prominent or large dorsum; stretch the bones or the nasal cavities. We can refine the shape and size of the nasal point an also change the angle between the nose and the lip (fallen tip). We can correct depressions or irregularities. In general we produce a harmonization of the nose on the face in every particular patient.
What limitations does the Rhinoplasty have?
The Rhinoplasty is a complex surgery. We can compare it to a 3D puzzle in which the surgery ”dismantles”, ”remodels” and ”reconnects” the pieces. It is not like a marble or wood sculpture in which you can evenly smooth until your reach the desired result. The severity of the alteration or preexisting dysmorphia affects what we can achieve with a Rhinoplasty.
We cannot and should not choose a nose from a catalogue or photo of an actor or actress. The intervention should not remove too much tissue to not weaken the nasal structure. There exists a limit in the capacity to adapt the skin to recover a nasal structure which has been made smaller, something we always evaluate. The type of skin, fine or thick and the age also influence the surgery. An finally the effect of the scarring is variable between one person and another, and is not predictable.
What are the risks with Rhinoplasty?
There is a slight risk of bleeding after the removal of the plugs which usually stops after a couple of minutes. And a slight possibility of infection in the suture stitches in the first week, without transcendence. The slight oedema of the surgery can hide irregularities or asymetries which can appear later. The healing process is essential and can also be the cause of alterations weeks or months after the surgery. These irregularities can be just palpable or also visible. There can rarely be a slight redness in the operated nasal area; this is more frequent in noses that have previously been operated on. It is important to realise that a surgery cannot guarantee a perfect result.
What is a ”retouch” of a Rhinoplasty?
Retouch surgery of a Rhinoplasty. If irregularity or an unprevented change in relation to the healing process is detected, which we think can be corrected we plan a refinement or retouch surgery. It is a minimal surgery, of short duration and recuperation. This normally requires a variable months waiting period to be able to carry it out without risk. In other occasions infiltrations of antiinflamatory preparations or substances of skin fillers can be carried out.
How will a Rhinoplasty affect my breathing?
The modern Rhinoplasty is designed to correctly maintain the nasal functions. In some cases noses that are curved on the outside can be obstructed in the interior of one or more nostrils, we therefore plan a rhinoseptoplasty or septorrhinoplasty to correct externally or internally the nose and the nasal septum.
This will improve the respiration and nasal cosmetics. Pinch noses after previous surgeries and noses that collapse when intensively inhaling can be reconstructed (secondary open rhinoseptoplasty) to improve the obstruction and the pinched appearance.

We will help you to make an informed choice, based on realistic expectations. We want to achieve the best result possible minimising the risks, this can sometimes mean choosing a more conservative intervention against a more ambicious one which could entail higher risks.
Each nose is different and that is why we look for a good result for each particular case. Our objective within the Rhinoplasty is the balance, the harmony and the stability with long lasting global results, with a nasal dorsum which is adjusted at the point, a nose that fits in with the face and which has the appearance of a nose which has not been operated on, and which is more natural than excesively small.
Do you need Rhinoplasty?
If you visit us with wishes to change specific characteristics of your nose and with an open mind, understanding the limitations towards what can be achieved; you could be a good candidate for a Rhinoplasty. But if you are someone who is obssesed with the shape of their nose, if you are worried about small or a mulitude of imperfections, or if you expect the surgery to produce a perfect result or that it will convert you into a successful person and will change the way other people see you, you are therefore not a good candidate for a Rhinoplasty. If you are confident that the planning that we carry out is correct, thought up to achieve the best result we think possible and share our expectations realistically, we will be happy to perform a Rhinoplasty on you.
What will happen in the first consultancy?
We think that it is very important to speak personally to the surgeon who will carry out your rhinoplasty.
We will explore the nose inside and externally, through an endoscope and palpation, ventilation manoeuvres etc.
We will take digital photographs for the preoperative analysis and clinical documentation.
We will revise the principal alterations which demand correction, and we will prepare a surgical plan for your rhinoplasty. We can use computerised analysis for the planning. It is important to know that the modified images are a guide and not a guarantee of the result of the surgery.
We will also give you a clear price estimate of the total process of the Rhinoplasty intervention. After this first informative visit you can decide when to be operated on by making an appointment for a second visit.
What happens after a Rhinoplasty?
At the end of the intervention a small splint is fitted in the exterior of the nose, on top of a small bandage. You will also be wearing a light nasal plug. It can be a day surgery or an overnight stay in the hospital. In 2-4 days the plugs are removed and in 7-12 days the external splint. The stitches will fall out on their own or can be taken out on that day.
We recommend no work for 2 weeks. And not to start sporting activity until 3 weeks have passed.
How will I feel after a Rhinoplasty? How will I see myself?
You will feel like you have a strong cold for 1-2 weeks, pain is rare, there is not usually pain. There can be bruising on the eyelids and slight swelling of the nose and face during the first 1-2 weeks of the post-operative period. The most notable swelling will be resolved within 10-14 days, afterwards you could feel a slight swelling sensation until 4-6 weeks, but only you can notice it. A lighter but more persistent oedema-inflammation on the nasal point can take more than 9 months to resolve. The final result can be evaluated when the healing process is completed, in a period of 12 to 18 months.
Who is my surgeon?
Dr.Ismael Garcia Roig is licensed in Medicine and Surgery by the University of Valencia since 1986. Carrying out his speciality in ENT (Ortorhinolaryngology and Cervicofacial Pathology) through medical intern residency for 4 years in the Clinical University Hospital of Valencia, ending in 1991. Since then he initiated vocational specialisation in rhinology and nasal surgery, attending to the operating theatres of the most renowned Rhinoplasty surgeons of this era and carrying out a multitud of courses.
He started international formation in 1997, carrying out stays in prestigious clinics of the American continent.
In the year 2002 he founded the Rinoplastika Clinic in Valencia, the first clinic specialising in facial plastic surgery. In 2005 he enrolled in the European Academy of Facial Plastic Surgery, where he has since been a numerary member. He collaborates with Dr. J. A. Mira since 2007 and with Dr. Nestor Asurey, both prestigious plastic surgeons.
For 4 years he has shared friendship and surgeries with Dr. José M. Battlés and more recently the growth of the project of the Institute of Facial Surgery Benidorm.
